Below are brief descriptions of each form, based on how and where it’s used in the process of planning and providing a workshop:
Studio Profile and Visit Notes – gather basic info about a new studio you’ve visited and to tell them a little about what you can do for them.
Free Workshop Invite – use this form after a cold visit to a studio (when you used the Studio Profile form) to follow up and allow them to choose which free service they’d like for you to provide.
Service Request Form – completed when a studio or gym reaches out to you and inquires about you providing a workshop or other service at their studio.
Event Workflow Checklist – a basic checklist to keep you on track throughout the entire workshop process from planning to presentation.
Seminar Agreement – our basic agreement form which includes the terms of how the workshop/seminar will be provided.
Seminar/Workshop Participation Waiver – our electronic form for parents or adult attendees to complete prior to an event. Waiver includes release of liability for participation, photo/media release, and activity disclaimer. A paper version of this form is also included (AZDM Seminar Sign In)
Injury Screen Intake – our basic intake form, including informed consent and photo/media release, for any onsite screens or assessments you may perform
Workshop Registration – our simple electronic registration form for larger workshop events
Step-by-Step: Injury Management and Prevention for Dancers – our basic injury prevention seminar which covers the injury experience, determining performance potential when injured, return to dance standards after injury, FAQs, and more. Included power point presentation, PDF copy for distribution, a notes card covering the important tips of the presentation, and an exercises card of some of our favorite warm-up exercises.
Event Survey – a simple survey form to get feedback following a workshop or event.
Therex flowsheet – a simple flowsheet for dancers to keep track of their exercise progress.
Injury Report – a report tool to help bridge the gap between the studio and the clinical office your dancer may visit. Includes reference to both dance movements for the dancer/teacher and anatomical descriptions of those movements for the clinician to aid in understanding aggravating factors and an accurate injury diagnosis. A PDF sample of this form is also included which shows how we break down our symptom questions.
